Why does my ex call me "miss smith" instead of just Lilly?

Me and my ex are still distanced friends - we speak on occasion. He has a strange habit of calling me "Miss Smith" instead of Lilly. It's not a sign of respect - he says it with a smug tone and a sly smile. I have no idea why he does this. He hardly ever just calls me Lilly. Any ideas?
